Notice Regarding Changes to your Cambridge Premier CAAP 403(b) and IRA Account

The purpose of this letter is to advise you that there will be a change of the Custodian of your Cambridge Investment Research, Inc. Premier CAAP Individual Retirement Custodial Account.

In accordance with the terms of Article IX Section 9.08 of the Roth Individual Retirement Custodial Account Agreement (the “Roth IRA Agreement”) or Article IX Section 9.08 of the Traditional Individual Retirement Custodial Agreement (the “Traditional IRA Agreement”) , and based on a request from Cambridge Investment Research, Inc. (“Cambridge”), BMO Harris N.A. (“BMO”) the current Custodian, hereby provides written notice to you of its resignation as Custodian of your individual retirement account as of the close of business, November 30, 2016 At the request of Cambridge, BMO hereby appoints Matrix Trust Company (“Matrix Trust”) as successor Custodian. Matrix Trust has acknowledged its willingness to serve as successor Custodian of your individual retirement account. Matrix Trust is a Colorado state-chartered non-depository trust company, regulated by the State of Colorado, Department of Regulatory Agencies, Division of Banking, and is a trust member of the National Securities Clearing Corporation.

Matrix Trust hereby gives notice of its intent to amend the terms of the Roth IRA Agreement or Traditional IRA Agreement, as applicable, effective upon Matrix Trust’s commencement as custodian. The terms of the amended Traditional and Roth IRA Agreements may be reviewed at If you would like to receive a paper copy of the amended Custodial Agreement, please contact ____________________________.

As a result of these changes you will be unable, for a period of time, to direct or diversify investments in your account or obtain a distribution from your account. This period, during which you will be unable to take these actions is called a “blackout period.” Whether or not you are planning retirement in the near future, we encourage you to carefully consider how this blackout period may affect your retirement planning, as well as your overall financial plan.

The blackout period is expected to begin on November 23, 2016 and end on December 22, 2016 During this time, you can determine whether the blackout period has started or ended by attempting to log into your account at or by calling Aspire Financial Services, LLC at 866.634.5873, option 2.

It is very important that you review and consider the appropriateness of your current investments in light of your inability to direct or diversify those investments during the blackout period. For your long-term retirement security, you should give careful consideration to the importance of a well-balanced and diversified investment portfolio, taking into account all your assets, income, and investments.

Once the blackout period has ended, you will still log into your account at , but you will need to use a new account number and a new account temporary password to access your account. A letter will be sent to you with your new account information and your temporary password.

Once you log into your account, please be sure to change your password.

If you contribute to your account by ACH, you need to make any changes to the way you submit contributions to your account. If you contribute to your account by check, please make checks payable to Matrix Trust Company and reference TPA 000207 in the memo line of the check. We have included a contribution form for you to complete and return.
